Grants to Massachusetts, New Hampshire, Rhode Island, and Connecticut nonprofit organizations for programs to address the needs of the aging population. Applicants must submit an LOI prior to submitting a full application. Funding is intended for workforce development, health and wellness, purposeful engagement, and field and capacity building.

The Tufts Health Plan Foundation supports the work of nonprofit organizations to improve, influence and ultimately create healthy communities and age-friendly cities and towns. The Foundation funds activities in Health and Wellness, Workforce Development, Purposeful Engagement, and Field and Capacity Building that will:

  • Improve or build systems to support healthy living with an emphasis on the systems that are serving older adults
  • Bring organizations together to collaboratively achieve broader impact and change
  • Scale efforts to address community needs

Health and Wellness

  • The Foundation is committed to community health, healthy living and disease prevention. It invests in programs and strategies that help older people manage illness and chronic health conditions, promote independence and help improve quality of life. All proposed activities should be evidence-based or evidence-informed, focusing on effective, science-based approaches that include rigorous evaluation and consistent outcomes. Activities should be a best practice or take a systems approach to expanding, scaling or replicating programs to older adults living in underserved communities in Massachusetts, New Hampshire, Rhode Island and, starting in 2019, Connecticut. The Foundation favors efforts to connect those in greatest economic and social need to resources. Efforts can consist of clinical and community interventions.

Workforce Development

  • People are living and staying in the workforce longer. The Foundation is interested in innovative approaches that address this new paradigm. The Foundation invests in training and job placement strategies that support older people in the workforce and create opportunities for older workers. Initiatives that allow older people to grow the skills and confidence needed to stay in the workforce help our country maintain the experience, talents, and insights built up over a career.

Purposeful Engagement

  • Older people are an asset to communities. They have experience and knowledge to share with others. The Foundation will support programs that provide older people with engagement opportunities within their communities and/or with other generations, provide a sense of purpose and alleviate social isolation. The Foundation also recognizes the essential role that family, formal and informal caregivers play in the lives of older people. The Foundation will fund engagement strategies that promote and support physical and mental well-being, including:
    • Engagement that alleviates social isolation
    • Volunteer activities
    • Intergenerational programs
    • Family and caregiver support

Field and Capacity Building

  • Systems change and collaboration can be challenging. The Foundation wants to address those challenges. The Foundation supports activities that build the field of healthy aging and strengthen the capacity of nonprofit organizations to enhance quality of life for people as they age. The Foundation also will fund initiatives that build field capacity including support for family and other caregivers in their essential roles as they care for loved ones at home. This includes training and technical assistance as well as planning, coordinating and collaborating around age-friendly communities. The Foundation will also consider capacity-building programs for nonprofit organizations that create a space for or expand older people's involvement in communities. Examples include encore careers, paid service work and/or volunteer opportunities.

Additional Eligibility Criteria:

The Tufts Health Plan Foundation funds projects serving communities in Massachusetts, New Hampshire, Rhode Island and, starting in 2019, Connecticut.

The Foundation does not fund:

- Candidates for public office
- Capital projects and/or equipment
- Development activities, such as fundraising events, capital campaigns, or annual fund drives
- Endowments
- General operating expenses
- Government agencies (unless within a collaboration led by a community-based organization)
- Individuals (including scholarships, internships, and/or fellowships)
- Lobbying
- Organizations that advocate, support or practice discrimination based on race, religion, age, national origin, language, gender, gender identity, sexual orientation, disability, veteran status or other characteristic protected by law
- Private foundations
- Religious organizations for religious purposes
- Research for specific disease states
- Underwriting of medical expenses

Organizations may submit multiple letters of inquiry (LOI), but only one grant will be considered per year.
